(for providers)

1/ What information should I include in referral?

A:  Please attach all prenatal records to date and a referral letter.

  • CPP

  • all prenatal bloodwork

  • all obstetrical ultrasounds

  • perinatal or antenatal record if done

2 / I belong to a FHO/FHT/FHN, will I be negated?

    A: The majority of prenatal codes are out-of-basket for FHO or FHN which means NO Negation.  Occasionally if a patient presents with an antenatal concern to labour and delivery triage and is NOT in labour but still requires FM-OB assessment an minor assessment code (A001) may need to be billed which can sometimes cause negation however if you indicate that your patient belongs to an enrollment model we will ensure this does not occur. 

3 /I would like to keep mom/baby postpartum, do you take them on as patients?

A: Our doctors are not able to offer ongoing family practice care to patients or newborns , but we will continue to follow them through delivery and see mom for a few postpartum visits  within the first 6-8 weeks. We will then return patients to their referring provider or other family physician.

4/ What  prenatal referrals do you not see?

A: We do mostly low-risk obstetrical care.   If your patient has twins, is a late 3rd trimester breech presentation,  requires a repeat c-section, has multiple or complex pre-existing medical conditions we may not be able to offer them care and would recommend you refer them to your local obstetrician/gynecologist.   We will triage referrals as they come in and respond promptly to let you know if we can or cannot accept. 

5/ Do you see patients without OHIP?

At present we are seeing mostly OHIP-covered patients, however we may be able to offer care to patients who do not have OHIP as well - these referrals will be assessed on a case-by-case basis.  Please be aware if patients do not have OHIP coverage or other provincial health coverage (excluding Quebec) a deposit that covers all physician services during their  labour and delivery will be required at first appointment. 

6 / What about general gynecology or contraception referrals or patients who are struggling to conceive? 

A:  Drs Swan/Tang/Prasad have an interest in women's health and are mostly accepting referrals for prenatal patients at this time.  However, we may accept referrals for contraception services, such as IUD or Nexplanon insertion, on a case-by-case basis.  We cannot do most general gynecology (e.g pessary care, fibroids, biopsies) or any fertility management.
